Hi, all you lovely doulas. I am a relatively new childbirth assistant and am building my repertoire of business forms. I need to work on a client evaluation form to use postpartum to get feedback on my effectiveness as a doula. Do you all have them? Anyone willing to share the contents? Many thanks, ~~Mary Reutener

Who are you training with? I use the evaluation form from CAPPA, my certifying organization. I know that DONA has evaluation forms as well, and some doulas choose to make up their own.

I have every client evaluate me. It not only helps me to keep tabs on my strengths and where I need to improve, but it also makes it easy when my recert time rolls around. I just pull 3 out of the stack and send them in with the rest of my recert. paperwork. :thumbs It's great that you're thinking on being evaluated on an on-going basis right from the start. :)

I'm making my own form, but I'm using this form to help me along, for inspiration :D
