I'm doing a babyshow on Wednesday and have a gift basket I'm going to do a draw for. But I wonder what would be good to put on the ballot if anyone has any ideas.
Besides, name and address and phone number and due date.

Email, Do you know what a Doula is?, Have you hired or would you hire a doula? Have you done any classes during pregnancy etc...

Use it as a way to get data about the women in your area

We just do name, phone # and email. I find more people will fill them out when they notice they don't have to give too much info. We had over 100 ballots from this past weekend's show!
