Photographic release/Permission form

Does anyone have a permissionform that they use for photos on their website and what have you and so that the client can be used as a reference? I would love to look at it or borrow from it if possible.

okay - Since no responses so far - Anyone have any ideas on what to say to create one? I have to meet with a client on Tuesday evening and I would like to be able to take it with me.

I did not have permission signed for the photos on my website because they are all of my sister. She knows they are there and I asked her, but figured she isn't going to sue me and if she does then we have more problems than that LOL. Actually my sister took those photos with the intention of me putting them on my website and on my printed material. She was very excited.
