Question labour log/progress notes

I'm trying to design a labour log/progress sheet (for when I have done sooo many births I will need it to remember!! )
Does anyone have a 'labour log' template I could borrow.
Or if not, what do you usually have on it; time, cxns, interventions, how mother coping..?
Any ideas would be fab Thanks.

I have one that is quite simple. I realized after attending my first birth that my jotted notes were hard to read and I didn't write the same things down each time. So I made up a very simple log to use during labor.

I posted about it previously here

I don't use a form, they make me feel a bit too constrained because there's not a place for everything.

I use a journal and I record vag exams etc, but I also take notes (always making note of the time) about the labor, how it's progressing, how mom is coping any funny statements or touching moments, etc. It makes writing their birth stories a snap. The other big perk is that I have notes from previous labors with me at births so if something crops up that I know we had a problem with in a previous birth but can't remember what we tried and what worked, I have it right there at my fingertips.

I do just like Angie, but I use a pocket size memo notebook, so I can have it and a pen in my pocket at all times. I write more that way.

Thanks Christy. Do you get the dilation/station info from the midwives notes I guess??

No...I just write it down when mom is checked. I keep my clip board handy so I can jot things down when the moment is right. I haven't had a birth where I was the sole support yet so it probably wouldn't be so handy in that case but so far I've needed to move away from mom when she is checked because hubby is supporting her for it and nurse/dr are right there so it has been the perfect opportunity to make notes. So, nurse on one side making her own notes and checking monitors, dad on other and doctor at the other end. lol
