I don't use a form, they make me feel a bit too constrained because there's not a place for everything.
I use a journal and I record vag exams etc, but I also take notes (always making note of the time) about the labor, how it's progressing, how mom is coping any funny statements or touching moments, etc. It makes writing their birth stories a snap. The other big perk is that I have notes from previous labors with me at births so if something crops up that I know we had a problem with in a previous birth but can't remember what we tried and what worked, I have it right there at my fingertips.
