Photography clause.

I want to add a section in my contract about photography. I want it to be simple like check a box or something but I"m not sure who to word it.

Do you have something about photographing the birth and using the pictures on your site in your portfolio things like that? What does yours say?

I don't have it in my contract.

As a photographer I offer those services in my intake interview and then if approved I take pictures at the birth. If I take pictures at the birth I use a photo release for my photography business and have them pre-approve pictures I can use by crossing out any they don't wish to be used on a printed contact sheet.
