Binder Form

I read in the binder threads that some of you ladies have the client sign a form, stating that if the binder is damaged or lost they will pay to replace it. How is your form worded, and how much do you charge to replace it?

I have a lending library contract which covers my binders. It says...

And there's a place to list borrowed books and then the client & I both sign it. Haven't had a problem (knock on wood!)
