Binder Form

I read in the binder threads that some of you ladies have the client sign a form, stating that if the binder is damaged or lost they will pay to replace it. How is your form worded, and how much do you charge to replace it?

I have a lending library contract which covers my binders. It says...

Please know that by borrowing any of my books, you are responsible for either replacement or the replacement cost if it is lost or damaged. And there's a place to list borrowed books and then the client & I both sign it. Haven't had a problem (knock on wood!)
