form listing interventions pros/cons?

I had a client ask me if I could bring a form listing all interventions and pros/cons of each. I know I have one some where but I seem to be very disorganized lately. She would like to make cards or something handy for labor/quick reference. She said she would really like to know what is truly needed vs. what the dr or nurse say is needed. She was told several times during her first birth that her baby would die if she didn't do x,y or z. She was afraid to question and wants to learn more of the facts for herself to be "armed" Thanks all.
