Evaluation forms

Do you do evaluation forms for yourselves?

I have the one I got from CAPPA that I"m using for now and it's working for now but I'm wondering what kind of form do you have/use?

I just use that one from CAPPA for clients. I explain to my clients that I'm already certified but that I will be required to turn others in when it comes time for my recertification. Each of my clients fill out an evaluation form, and so I have a good pile to pick from when sending in my chosen 3 for recert.

My favorite part of the form is the 'additional comments' section. I've been doing evaluations this way for the past 4 1/2 years, and it's worked well for me.

I use my one from DONA for re-certification purposes with each birth. There is also a great evaluation form Christy (MotherTheMother) here in alldoulas has kindly given to me and I use that also. It's more in-depth and detailed and also has releases on it for photos, website useage.

I have the ALACE form, but it isn't in depth enough for me. I have created a pretty elaborate one, but I have recieved everyone back so far and they are so helpful for me! I get to know what I could have done better and I get some great tesimonial for my website!
