I would like to include something within the body of my Confidentiality & Release Form about photography.

I don't usually take photos during the birth, but I do tend to take a number (with my camera) after a birth. I would like to be unrestricted to use those as I wish.

What one or two line phrasing can I use to succinctly state that?

Legally the pictures are yours anyway. But since birth is such an intimate environment it would bode well to have some sort of a release.

Personally, I use the same release that I use for my photo business, and I print a contact sheet of all photos I'd like to use and attach that to the signed release.

If you google photography release you'll come up with some samples that you might be able to to strip down into something that suits your needs.
