Release for pictures?

For those of you who take photos of the births, do you have the parents sign any kind of release for those? I'm just starting to add maternity and baby photos to my services. I'd been doing birth ones already. But with the maternity and baby ones, I wanted to do a portfolio to give my clients an idea of the type of photos I'm shooting for. (I probably would not include birth ones since those are so personal.) I am very much an amateur at this. If they give me permission to use their photos in my portfolio, should I come up with a form for them to sign? And if so, what should it say? Anyone have one to share???

I have just a basic phot release form. Since I wiped my computer I don't have the file on here any more . . . but that gives me a good reason to scan it in again! If I dont get it done soon, pm me! I think it is actually from DONA though? I use it cause I put baby pics and such on my website, but only if the clients are comfortable with it.

Photo Release

The client(s), ______________________________ and ________________________ give permission to the doula, ______________________________ to take pictures at the birth of their child. It is understood that the pictures will remain private between the client(s) and the doula unless permission is granted otherwise.

The client(s) (do / do not) give the doula permission to show just one picture of the mother and child after the birth to her new clients.

The client(s) (do / do not) give the doula permission to show future clients any and all pictures that were taken during the birth. These pictures will only be shown as a means of explaining to potential clients what kinds of pictures the doula takes during her work.

The client(s) (do / do not) give the doula permission to show a picture of the mother and/or child after the birth in her baby photo gallery on her website.
