Doula receipts

I am new to this site but am wondering if anyone out there has a good doula receipt template that they use for clients needing a receipt for insurance purposes. I have 2 recent clients who are paying me out of there medical savings account and need a paper receipt to back it up. Any help would be greatly appreciated! THanks so much!

I bought a receipt book at Walmart with a carbon copy. It's worked great for me but I don't know if the insurance company would accept it.

I have a couple that I could share with you. If you PM me with your email addy, I will send them to you via a Word doc attachment. If you like either or both of them, then you can modify them through MS Word to suit your needs.

I'll add that I've had clients use my receipts to submit to their Flex Spending Accounts (medical savings) through their employers and had no problems at all getting their money. I've not had anyone submit them for traditional insurance coverage yet, though.

I just did up and invoice in WP and keep a copy for myself and a copy to the client.

i might mention if they are going to send it to insurance companies you might look into any sort of special coding they want in order to make things go smoothly.

Absolutely. One of the forms that I shared has the diagnosis code and CPT code. I have suggested that whoever uses it to check to make sure that they are still the correct ones. I haven't used that particular form in a few years, and I know that for whatever reason, sometimes the codes can change over time. I am not guaranteeing that they are presently correct.
