where do most of you get your forms

I've started reading the binder info...but as to contract and waiver type forms, do you create them yourself, use that Doula Office program (which I'm considering) or borrow from each other??

Mine is a mix, from borrowing, a cd I bought on ebay (not doula office, but I REALLY want to buy it) and from throwing in things myself. You are welcome to it, I can email it to you.

I bought a packet from my doula trainer when I was first starting out.

I make almost all of my own and use a few others like Penny Simkin's pain scale. There are some fantastic tools out there and I don't see a benefit to trying to reinvent a really well created wheel.

Mine are also a mixture of ones I've made up, doula office, and ones others have been so kind to let me use. Are there any in particular you are hunting for?

None particular, just ready to get rolling, ya know? I've got the blog together, and now I want to work on the other business stuff since it is comfortable to me. I'm really enjoying reading but some nights I just have too much energy to sit and read.

I've bought tons of forms in my career and what I use now are a mesh of different forms that had ideas I liked and needed and I took what I liked the most and created my own forms that are tailor made for my practice.
