Without having seen the pictures on Vista Print, here's my : You can do whatever you want as far as the picture goes, IMO. I have a line drawing of a baby in a basket with stuffed animals looking in that was modified by a graphic artist especially for my cards, but I had my business cards done at a local printers. It's pretty neat...I had it done with thermography (raised ink). I've seen it both ways, some doulas having a baby, and some having a pregnant belly. My advice is to go with what your heart tells you about what you like and what you want. Just ask yourself if it represents your services or not (and go with something you like that does). I just LOVE, LOVE, LOVE little babies, so I couldn't help but to lean that way. You don't have to do everything just like other doulas that you see. That's the beauty of this business; there's a lot of room for individuality!
Oh, my vote is to not have "gentle" in front of hands. To me, it feels more awkward to say.
