I've done CBI, but I also took a workshop by CAPPA. Honestly, I like them about the same - but for different reasons. I originally chose CBI because I didn't have to recertify or pay for continuous membership, I didn't have to attend a workshop, all the material and submissions you needed to make were online, I'd heard it was very in-depth, and it seemed less "formal" and more personal than a larger organization, if that makes sense. I think a con to CBI for some people would be its lack of recognition in some communities, and the lack of hands-on and networking opportunities as they don't currently have a conference or any workshops. If that's important to you, you may prefer CAPPA.
The CAPPA conference was in Nashville this year, and I decided to attend. I took DoulaVal's training, and it was really informative, even though I knew a lot of the information from my CBI training. She covered a lot of marketing stuff, I was able to better understand the more physical comfort measures, and I met a lot of great ladies. Workshops really do have a lot to do with your trainer, however, and Val is a great one.  Everyone said CAPPA was like a big family, and before I attended the conference, I thought they were just being cheesy. But it really is. It felt very comfortable and laid-back, and everyone was great. I'm going to cross-certify with them once I get my CBI certification over and done with. I love it!
