I found this book - you can only get it through the writer. Someday I hope to order it. It is called Birth Guardians - Here is a description from the writer - The contents of the book are as follows. This may or may not fit but
perhaps we could do a collabortion with some added pages for chart
notes etc.
The page for each birth contains a section at the top for name, age,
gravida, weeks gest., time and date of birth. Baby's name, sex, wt.
length, presentation,, other children and names, care providers,,
apgar, place of birth, time from onset to completion of birth,
time/date care began, total attendaence time, # of prenatal visits, #
of postpartum visits, etc. etc.
Then the is a lengthy part of the page for the birth story. I could add
in back and front lined pages for charting etc.
The next section is for gathering stats for the births recorded in the
book. there are 58 fields to fill in for each birth attended. It is
layed out to give you as complete overview of your personal stats.
SO...when a client asks, how many c-sec, tears, hemorrhages, inductions
etc. Whala, there it is! The are also sections for contacs, keeping
track of continuing ed etc.
