Page from DONA training binder

I was wondering if someone could help me out. Page 95/96 from my training notebook got torn in half and pg. 96 is one I use all the time (It is the "Questions for Informed Consent or Informed Denial" page)

I wasn't sure if all of the training notebooks were same at all the trainings or if they were different depending on whose workshop you went to. Does someone have this page that they could send copy and send to me? Is there a place where I could get it online?
