If you have your clients fill out an evaluation after the birth, when do you give it to them? Before the birth so they have it? At the postpartum visit? Do you give them an envelope with it? Thoughts? TIA
I usually give it to them at the post partum visit, and I tell them I don't need it right than and they can mail it to me.
I also bring a sase for them just in case they do just want to mail it back to me instead of filling it out right there. I have found that most want to just get it finished and give it right back to you.

I either had them do it before I left the hospital (in these weird cases they weren't doing anything but chatting with family or friends and offered to do it before I left) or I took it to the hosp the next day when I visited. In one case I gave it to mom with SASE and it took her FOREVER to get it back to me. I don't like doing that cause I know they have lots on their plate with a new babe at home and it can understandably get lost or forgotten.

I ususally leave one with the parents right before I leave the hospital and tell them that they don't have to fill it out right away, but that I'd like to pick it up at the postpartum visit (approx. 2 weeks after the birth). I make sure that mom and dad both know about it and of my intentions of getting it back at the pp visit. It's worked every time for me doing it this way!
I give the evals to them during our prenatal visit (with a SASE (under the contracts section in their notebook) and tell them I would like to collect it at the time of our postpartum visit. I usually remind them I would like to pick it up when I call to confirm our prenatal appointment. This gives them time to reflect on their experiences. I have gotten some really thoughtful responses that have been quite wonderful. I have even used some of them as testimonials on my website.

I do it at the postnatal visit and leave it with a self addressed, stamped envelop so they just have to pop it in the mail. I also have written on mine that any comments made, may be used as a testimonial on my website.
