PPD Contract and Abbr. Info?

Does anyone have a copy of a PPD contract I could see? I have been contacted about possibly doing some PPD work for a couple I taught for but I want something in writing. Also, does anyone have a great one paragraph explanation of what a PPD is/does? TIA!
