How does it change your practice?

I mean...if you are a volunteer, or working under grant funding or insurance funding, how do you find that it changes how you doula?

Personally, because I'm a program doula funded through grants and insurance, I am less apt to speak up to doctors, or to present information that goes counter to what the doctor is saying in front of medical staff. My concern being that I don't want it to reflect on the program negatively, frustrate or piss off the staff, and possibly create some conflicts with insurance not being happy with how we practice. Overall our organization is very clear with doulas to be careful what we say to our moms because if it doesn't appeal to the CP, then it gets back to the org, and possibly back to the insurance. This is also why I don't like the idea of doulas being reimbursed through insurance, because we become responsible and may have to face restrictions in our practice as it comes down from insurance.
