 Insurance Re-imbursment

Sorry my spelling is terrible. I applied and received my NPI number. I have a client who would like to submit her claim to insurance. How do I get the form to fill out so she can send it to her insurance? Any advice would be greatly appreciated. Thank You

Try this link for the article from ICEA. Scroll to bottom of page for info on documents to use until the actual doula forms are ready. I haven't heard more on them.
I'm in the process of filling out the form for my NPI. I'm awaiting help from NPPES...7business days they say.... I am just so excited about this possibility! I'd love to hear how it goes for you! And any info anyone else has around this topic.

i have heard that clients can use their own insurance claim forms with a reciept from the doula with her number and the codes..

I just posted not too long ago about one of my clients who got full reimbursement. I give my clients a receipt with the diagnosis and CPT codes and a how-to sheet on how to get reimbursed. They just send it in with a claim form that they can get from their ins co. I attached a copy of the receipt I use to that thread - it's in general doula forum iirc.

Does anyone have a copy of a completed sheet that they would be willing to share for reference? I really want to see what this looks like. You can omit client information, obviously. PM me if you can email me a copy.
