i have been stalking this site for almost an hour now and i have read a lot of great things but i am kind of overwhelmed lol
i am wondering what all you use for your clients as far as forms are concerned.
i am going to put together a client folder which i think will include: (after they hire me)
-an about me page
-services they get
-placenta services
-a birth plan
-a postpartum plan
-various information
-my contract
and....?
this where i need help...do i need to have other forms like any other consent forms or information etc?

I broke down and bought doula office. And i'm glad i did!
Before I'm hired, I give potential clients a free interview meeting. To this meeting, I bring 2 folders.
In 1 folder, I put the stuff for the potential client:
Contract, HIPAA form(functions to make it a little more official in my mind, plus the fact that I will be transmitting their health information to my certifying org), SASE, Contract mailing instruction page, and needs assessment, plus my business card and a business card sized card about the birth survey.
In the other folder, I put stuff for me/stuff I might want to give my potential client if something arrises:
Client info page, doula support log, application for alternative payment, payment plan form.
I also bring along the binder that I loan out my clients, just in case I want to whip it out.
My contract has all of the info about what services they get, and we talk about birth plans in the first prenatal. After they hire me, I give them a couple more pages at each prenatal. IF, for instance, a potential client mentioned something about her DH not being completely on board, I might include a dads and doulas page, or a page on how doulas help.
